Please fill out one form per student. The deadline to submit this form is extended to thru Saturday, February 28th.

1.  Submitting this form will confirm your child's participation in the End of Season Showcase 2015.

2.  Submitting this form will confirm payment for the Facility Fee (per family) and Costume Fees for non-company dance classes.

3.  Submitting this form will confirm participation and fees for the 2015 Musical Theatre presentation: ROCK of AGES.

My child will be performing in the End of the Season Showcase 2015 on SATURDAY, JUNE 27th at the Hunterdon Central Regional High School Auditorium.
The Facility Fee insures that all families share in the cost of the theatre. It includes 4 tickets total and 2 special gifts for your performer(s). More tickets are available if needed.
NON COMPANY student's parents please check the appropriate boxes to confirm payment and costumes to be ordered.
COMPANY dancer's parents please check below to confirm non-company dance participation.
MUSICAL THEATRE PRESENTATION 2015: ROCK of AGES
Check below to confirm your child's participation in the Musical Theatre presentation. A Costume rental fee is required by all persons performing. All Musical Theatre performers must also cover the Facility Fee listed above.
